Interventional procedures like taking tissue sample from tumor, taking out fluid accumulated in chest wall or abdominal cavity, aspirating pus collected inside the body are safely done under ultrasound guidance where we can see the needle path.

For abdominal sonography it is preferable to come empty stomach for 4-6 hours. Patient can drink water. In case of emergency it can be done without any such preparation.

For pelvis and kidney sonography patient has to drink 4-5 glasses of water one hour before sonography and patient should not pass urine for one hour before coming for sonography.

2D Echo with Doppler is an examination that is performed on the ultrasound machine to detect abnormalities of the heart. It is used in many situations to detect cardiac function and ejection, valve motion, as a pre operative fitness test, to look for fluid around the heart, and motion abnormalities.

WHAT IS COLOR DOPPLER SONOGRAPHY?

In color Doppler the blood circulation is assessed. It is more often done in diabetic patients, in chronic smokers who are more prone to develop blockage in their arteries, in patients who suffer from varicose vein problem.

It is non invasive test which tells you about blood circulation to any particular part of the body like upper limb, lower limb, blood supply to any tumor or swelling involving the body.

Carotid Doppler is done to check for blood circulation going towards brain.

Peripheral and carotid color Doppler does not need any special preparation, except for renal Doppler where patient has to be empty stomach for 4-6 hours.

WHY SHOULD IT BE DONE?

Mostly babies are healthy, but sadly some have problems that could be serious. The prenatal screening tests allow an early prediction of problems and segregate the high risk cases which will need further tests.

FIRST TRIMESTER COMBINED SCREENING

It is a combination of fetal ultrasound and maternal blood tests. The test includes:

ULTRASOUND TEST: This is done at 11-13⁺⁶ weeks and is performed by FMF (Fetal Medicine Foundation) certified operator. All the three radiologist at shobha diagnostic centre are FMF certified. This involves evaluation of Nuchal Translucency (fluid behind baby’s neck) and other markers for chromosomal abnormality like nasal bone. These measurements are abnormal in Down’s syndrome. The overall fetal anatomy is also checked at this time.

BLOOD TEST:  The blood test performed on FMF approved bio chemical assay systems measures two hormones—beta HCG and PAPP-A. Abnormal levels of these substances are are indicative of enhanced risk of chromosomal abnormality.

WHAT IS DOWN’S SYNDROME?

Down syndrome is a chromosomal genetic disorder in which the affected person carries an extra copy of chromosome 21.

It is the most common cause of intellectual disability in children. They also may have other health problems like heart defects, intestinal problems, hearing and visual problems.

 Can Down’s syndrome be cured?

No!!! There is no cure for Down’s syndrome.

 Can Down’s syndrome be detected before child is born?

Yes!!! By doing above mentioned first trimester screening it is possible to identify pregnancies that are at a higher than average risk of having a baby with Down syndrome. Then such mothers can undergo amniocentesis test in which fluid around the baby is sampled and sent for chromosomal testing (Karyotype) which will detect the extra coy of chromosome 21.

All the superficial parts of the body like neck, thyroid, orbits, breast, any soft tissue swelling are best assessed on ultrasound.

No preparation is needed for this study.

In today’s era of advanced technology there is dramatic improvement in our understanding of birth defects in children. Approximately 3% of new borns have a recognizable major anomaly and its disheartening to know that birth defects are most common cause of mortality in children in our country.

Systematic fetal sonography is the first step in detecting and diagnosing the birth defects, however to diagnose chromosomal abnormalities and certain hereditary syndromes more advanced tests are needed.

It is more advantageous to prevent occurrence of a genetic disorder, diagnose and treat it at pre clinical stage than to wait for the birth of a baby with serious genetic disorder.

There are very few centres in Western suburbs who offer Prenatal Diagnostic Procedures (Amniocentesis, Chorion Villous Sampling-CVS, Cord blood sampling, Foetal reductions in multiple pregnancy) and Shobha diagnostic centre is one such centre to offer these services.

• Amniocentesis: Removing amniotic fluid around the fetus and sending it for analysis. Done at 17-22 weeks
